Authentication Error while adding User Directories

I am trying to connect Crucible to the JIRA user directory for the user management, so that I can import the users from JIRA to crucible,  while adding the directory I was asked for Application Name and Application Password, can someone please let me know what should I enter in these fields. Please check this attached screenshot.

Go to JIRA, then user management. Select JIRA User Server, then Add application.

What would be the IP Address?, is it going to be crucible IP address?
