Associating an issue type to a screen

I'm trying to associate an issue type to a screen I have created. Within the screen I have added in custom fields. I have then added the issue type to the project, but when switching to the issue type, the fields on the create issue form remain the same. 

I have followed this guide, but it still doesn't appear to be switching to my desired form. 

 

Any help on this would be greatly appreciated.

Hi Bryan, 

 

Thanks for your prompt response. I believe I have figured it out, myself. I added the issue type screen scheme in the screens section of the project, and it appears to switch to the desired screen, when I select the issue type.
