You are in the System Field right (Administration > Issues > Issue Attributes > Resolution) ? Scroll to the bottom most part, there should be a "Add New Resolution" section in there where you can add new resolutions.
So how does this help with adding a resolution that already exists?

As people have already mentioned, this Resolution already exists - and if you see it already in the Resolutions page, then something is hiding it from the drop down menu. If the resolution is not in the page, though, maybe someone removed it and you'll need to add it back again.

Right, that sounds like someone has used workflow properties to suppress it. Adding another one is the wrong thing to do (and it won't let you duplicate names). Check the workflow for the issues it does not appear for and look at the properties of the transistions.
