3 date fields, added in a request type, are not shown in request form

We have 3 date fields - custom fields, mapped to a project, added in a request type.

 

but, in request form it is not showing. Please guide us.

It is a different issue

I have 3 custom fields. All these 3 are date fields, mapped to 2 screens. One is a default screen, other one is tab which is used in a transition

 

In tha tab screen, it is showing. where as in the create issue issue screen, which is the screen where the user is submitting the request... these 3 fields are not shown for data entry.

I checked with other date fields with same scenario. Those are working fine.

Kindly make sure, that the fields are present on the view screen for the required issue type.

In create screen itself, these fields are not visible. All 3 fields are Date type.  

during a transition, we are showing a screen, there it is showing
