This can indicate a damaged issue in your index or database.

Can you go to Admin -> Integrity checker and run the checks related to workflow?  If they find errors, tell it to fix them.

Ok, we need to read your logs to see what is crashing the integrity checker.

Look in <jira home>/logs/atlassian-jira.log and look at the time the integrity checker last crashed for you (or run it again while tailing the log).  It should have a lot more detail.
