In our system, we have a special button which is tied to Jira (clicking on it, Jira window opens). When reporting a bug and clicking on "Submit" the window becomes frozen and there is only one way to close it is to click on "Close". The issue is sent to emails, but from the user side, they do not see any notification that the issues have been created. It causes a lot of duplicates issues are created.
