Form Application Recommendations

Currently we are running JIRA and Confluence, and looking into the applicability of Service Desk to our institution. For forms, we are using Frevvo Live Forms. The integrations between Frevvo and JIRA is shaky at best. We are exploring other options that integrate well with JIRA and Confluence, as well as pass WCAG 2.0 accessibility requirements. The form software must also have workflow functionality, and be able to be self-hosted. Any recommendations on what people are currently using, or thoughts on alternative products?

Hi @Brandon Viertel, have you explored Jira's built-in Issue Collector functionality?  It allows you to embed a Jira create form on a website.  When a Jira user completes the form, a Jira issue is created and is associated to a workflow, just like any other Jira issue.

The Issue Creator has some limitations, but after you paste in a small bit of code any changes you make on the Jira end are automatically reflected in the web form.
