edit service desk request forms

Hello

We'd like to edit service desk request forms to logically divide fields in a single request, e.g. User Account Request, User Software Request, User Hardware Request. 

This would logically group the fields in the request form and make the form overall more readable as we would avoid confusing the end user with similar looking field names.

To our knowledge, this can't be done with only fields. We've tried to inject custom JavaScript into forms, but this doesn't work well as most plugins evaluated load scripts on page load; elements referenced in the script haven't been rendered.

If anyone can recommend a plugin, or a technique using the base software, that would be great.

If you're open to using a plugin, I would suggest ProForma, which was acquired by Atlassian some time ago.
