Unable to connect Slack to Jira Cloud

Hello,

I'm trying to use the Jira Cloud Slack App, but I'm unable to connect a channel to a project.

• I have admin access to both the Slack workspace (Owner role) and the Atlassian Site (Org & Site admin)
• The bot appears in the list of Apps in the Slack sidebar
• Trying to invoke the `/jira connect` command just displays "Something went wrong" message
• Trying to invoke the `/jira create` command prompts a popup with no projects in the drop-down menu
• There's no sign of the Slack integration anywhere in Atlassian/Jira Cloud
• Visiting https://{my_company}.atlassian.net/plugins/servlet/ac/jira-slack-integration/addon-config-page URL shows "Something's gone wrong" error popup (image attached)

This could be an issue on either end (Jira or Slack). Just for general reference, here is the integration documentation from Atlassian. It might help triage the issue.
