On the Plan view of the JIRA Agile board, there used to be a box with a plus sign underneath the backlog that would allow users to quickly enter an issue without going through the Create dialog box. It disappeared sometime in the past week or two. Where did it go?

Yes, that is a bug introduced on the release of November 3rd. Here is the link to it: https://jira.atlassian.com/browse/GHS-11418. Please feel free to add your comments and vote for it to make it more noticeable to our developers. Also refer to Implementation of Bug Fixing Policy for more information on Atlassian's approach to the development of these bugs fixing.
Until a fix is released, you can still use the workaround provided on the bug report.
