We created a business project in Jira using the Process Management template. We have customized the workflow and one thing we do in most of our transitions is prompt the user for non-required comments. This works fine in our software projects. We followed https://confluence.atlassian.com/jirakb/how-to-add-a-comment-during-a-transition-779160682.html to get this to work in the software projects some time ago. Using the same Comment Screen does show a prompt, but there are not any fields in the pop up.
