Multi-page requests in Jira Service Desk

Hello

 

I have a client request to create a multi-page request form. The form is in three parts, with navigation (forward/back arrows at page footer) and headers indicating the particular part of the request the user is at, e.g. "Onboarding" on page 1, "Hardware request" on page 2, etc.

See below. Does anyone have experience with this kind of setup?

This would only be possible by developing your own code to configure and display multiple pages. JIRA Service Desk doesn't support this out-of-the-box.
