How can I add a reason (description) to a flag in the "new generation" projects?

Before the change, the product had a very ugly way to add specify a reason to a flag (the reason went to the comments as flagon and flagoff). It was very ugly and very hard to do some qualitative risk analysis on blocks, but it was possible to do. How can we do it in the new generation? Instead of improving an important necessity you simply removed it? How do you manage flow? How do we run risk reviews on blocks now to manage actions to improve flow?
