It's common to see this dialog box for a split second until the edit screen loads. However if you are not seeing the edit screen appear, and only this screenshot view is showing on the screen, clearly there is a problem with loading this edit dialog box on the screen.
I have seen other Jira instance that have had similar problems caused by certain specific plugins to Jira. As such it might be worth trying to narrow down the scope of this problem by trying to place Jira in safe mode. This allows you to use Jira without user plugins. If you have a lot of plugins it could take some time to narrow down which of these might be causing this.
The other alternative way to try to troubleshoot this is to recreate this problem then immediately look at the $JIRAHOME/log/atlassian-jira.log file to see if there are any kind of errors being thrown in the logs when this happens. There might be nothing, or there might be a clue here that indicates if a specific code package is trying to be called.
