Hi team,
We are trying to add Form "New Employee Onboarding Request" template under HR.
After adding we are not able to save it, please let us know if there is any issue or this template is under development.

Testing on my local site, I was able to replicate the same issue and this error happens due to the Email field on the form.
Please, remove the Email field that is under the Business cards section, and then, on the top click on Fields > Email to add the field again and just edit its name on the right side.
After that, click on Save changes and the form should be saved normally.
