I would like to create a ticket in MS teams where anyone in our organization can submit a request using that form, and that submission then creates a task in Jira through their integration.
Another alternative is if anyone emails a designated outlook email that is entirely for submitting requests, once an email is received it triggers a task creation in Jira.

You have a few options depending on what you want to achieve.
Jira Cloud for Microsoft Teams:Streamline your workflow with custom, personal notifications, link previews, and the functionality to create, search, and update Jira issues—all within Teams. You can get it from Microsoft AppSource or the Atlassian Marketplace.
Jira Cloud for Outlook: Create new Jira issues, view up-to-date information about existing issues and take action on them in a couple of clicks. (Microsoft AppSource link)
Zapier's Teams Jira Automation: When X happens in Teams, Y should happen in Jira.
Then there are a range of other options on the Atlassian marketplace if you just look for Teams Integrations.

I'm Benjamin from yasoon, we are developing app integrations for Microsoft 365 to Jira/Confluence and actually offering a wide set of possibilities to connect Microsoft Teams to Jira and vice versa.
If you are working in a Jira Service Management context you could use our Portal integration in Teams that will reflect your native JSM portal directly in Teams.
Additionally we provide automations that can help you speeding up your task management, for example: whenever a new issue is created -> post something into a teams channel or when a keyword is mentioned in Teams -> do something in Jira.
