Is it possible to show fields based on workflow status in Jira Service Desk Customer Portal?

Example:

I would like to show the change start/end date once we review and plan the change. So customers will not see these fields during change request submission.

No, field displays are not dependent on status, they're dependent on whether they are on the view screen or not.
