I have example with a change request type where the customer form to submit a change request doesnt allow the fields of Change implementer or approver to be populated with a user who is not part of the Service Desk team.
It appears that if the change implementer needs to be assigned, then they must be part of the Service management team. Is that correct?

That's correct. Licenses in Jira Service Management are counted towards the number of agents you have. That's the list of users who are part of the "Service Management Team". Only those users are able to work on issues, respectively to become assignee.
In order to give users permission to raise requests, you simply have to put them in the "Service Desk Customers" role.
