Jira Service Management 4.5.10

On the customer portal I would like to display a field presenting a list of services.
When the customer chooses a service, then it must be possible on another sub-field to choose a sub-service.

example

Type of service (e.g. Management)
Perceived service (e.g. SAP)
Service delivered (e.g. SAP ECC)

The idea is to let the user set the first two and then, on the jira Service Desk Agents (IT) side, a final, third list field (which the customer does not initially see on the customer portal).

Depending on the Request Type, I would like a specific service list to appear.

For example, "report a problem" under the "Applications" group should only have Sales, Accounting, Documentary, Factory, Management, Warehouse and Miscellaneous as Service Type.

In this logic, for the groups "Workstation" and "Reports and BI" the Service Type, being unique, would be nice to be predefined (or simply hidden and managed with automation).

Can you suggest a solution that does not involve Insight? Because I can't buy Insight anymore, as I have jira server and don't want to buy jira datacenter right now.
