I have created a portal with service request types. When the customer opens one of the service request forms on the portal and after filling in the required information submits it, can it be automatically mapped into a change request.

From Service Project -> Project Settings -> Request Types page, you can simply map request type with issue type. So, request, submitted from the portal, will be created as change request in the system and follow its workflow.
If you would like to change request type (from one to another) after the creation depending on some criteria, you can create automation from Service Project -> Project Settings -> Automation page.
