How customize the form the services

Hi everybody, I'm tryng to add fields in the form about "Services", by default this form have a "Tier", "Change Aprover" and other fields, but I need to add other specific to my company. It is posible in Jira Service Management and where o How? I can't find a reference abouts this operation in the documentations.
