Welcome to the community.  In Jira/JSM Cloud env, it is not possible to achieve what you are asking for.  The only thing that you can do/control in Request Type Form configuration or Field configuration to add your custom help-text (i.e. tooltips) against the fields.

Note - Custom help-text setup in your Field Configuration Scheme will only be displayed in Issue Creation/Issue Editing screens.  Those help texts will not be available when one viewing the issue.
