Form/field design - can it be done?

Using Forms in JSM, I'm looking for a way to capture (and later export or report) the following scenario:

Customer should be able to select one or more Supply items, and for each item selected indicate the amount they currently have on hand.

For illustrative purposes, lets say there are 10 items - A-J.

• customer selects Item B and indicates they have 1 on hand
• customer also selects Item E and indicates they have 0 on hand

The only way I can picture accomplishing this is to create 20 custom fields - one field for each item (10) and one field for each corresponding on-hand count (10).

Is there a different way to configure and capture this using one, or at most two, custom fields?

Hi Darryl,

You can probably do something like this using Assets. Since you already have a Premium subscription, that gives for "free".
