Jira Service Desk - Server
We provide an online Medical Billing solution to medical practices and billing services. Billing Services will have multiple practices under their purview. Each practice has a database, so a billing service will provide services for multiple databases. In order to assist our clients, we need our Support Team to know which Client and which practice the client user is reaching out from.
We would like to have a contact field that would allow us to select a database number, which would then auto populate the Client/Company as well as the practice information based on the Group Contact information, along with the list of users specific to that client. So far I am not able to find this functionality within the Service Desk.
Is there a way to accomplish what we need?

Consider using the Insight - Asset management app.that adds the capabilities of storing and using master data.
This will allow you to keep your customer and all metadata around your customer ( database numbers, customer contacts, shipping addresses, agreement, SLA etc you name it! ) in one place and in conjunction with your Service Desk requests.
Depending on what customer that adds the request, the post functions that comes with this app can find and attach the needed information to the service desk request giving your agents all needed context to work efficiently and effectively.
