How can we automate the process of workflow in a service request/incident or change management.
eg. we have created an incident and we want that statuses shall automatically been changed when a comment has been added and notified the approver to approve/reject the request.

i do not really understand why you have an approval within an incident. That is something within a request for change or service reuest with approval from my point of view.
Anyway, have a look at the getting started guide. Your mentioned Use Case is very easy to implement and the creation is very intuitive from my point of view. ( until you come into more complex use cases with smart values, lookups and so on…)

You should look into the rule templates. There are some of the most common szenarios.
