Pre-selected form content in Jira service desk form

Hi,  

I have a bunch of forms that I have designed in JIRA service desk.  Some fields have multi selectable items either via selecting checkboxes or using the multiselect/dropdown options.

I want to present the requestor, with a form with some content already pre-selected and they can de-select if required.  

How can I have form content already selected?

This widget could not be displayed.

Or is there a way to add a specific piece of text to each issue created saying something, like instructions for the person who will be dealing with the logged issue?
