Jira service desk - required fields showing up multiple times upon issue creation

Hey guys,

 

Hoping someone can help me out! I created a project in Jira service desk and added in some custom fields so that when a user logs in and creates an issue, they first see the required fields for them to fill out. For some reason, we are seeing the field show up multiple times. Any one know any reason why?
