Issue Collector to embedded with Service Desk portal

We have configured JIRA Service Desk and would like a feedback button embedded in the customer portal so that users can send their feedback on the quality of the service our Service Desk provides.

Upon setting up an issue collector, JIRA provides a snippet. Where can this snippet be pasted so it displays in the portal ?

Goto: https://marketplace.atlassian.com/plugins/com.cprime.jira.plugins.surveyproject this does exactly what you want. You have to pay for it though....
