We are evaluating Service Desk. We are interested in creating an access request form that would be filled out someone (hiring manager, other person), then approvals are sent to application owners. When approvals are completed, tasks are created to have IT staff grant access. When all access is created and the sub-task is closed, the form is closed. Is it possible to do this? Which software would be needed for this?

Great question. You would be able to configure your customer portal to show a request type that would lead to a form, configure your issue types and create workflows that would consider your multiple approvers for that request.
You would be able to automate your service desk using a variety of triggers/events shown in that table out of the box. Create sub-tasks specifically based on an action or event might be best done with an add-on from our Atlassian Marketplace such as Automation for JIRA.
