Creating issues on my website

I have my own corporate portal and JIRA service desks (on another server). I want the tasks/issues in the JIRA service desks created from the sent form from my site.

That is, it filled in the form on my website -> sent it -> in JIRA created task/issue based on the data from the form.

Is it real? How can i do thiis?

Is your website sending an email when this form is completed? If so, then it should be possible to create issue in Jira or Jira Service Desk if sent by email provided that you have created a mail handler in Jira to check a specific email inbox for new messages.

The other way you might be able to do this would be through the use of an Issue Collector.  This is a HTML/javascript form that you can embed on an existing webpage that provides users a number of fields to complete that in turn creates issues in Jira.
