If not, is it actual included in the actual Service Desk Server Version?

Yes approvals are available in all the Jira Service Desk versions since at least 3.2.x and higher. But these have to be configured in your environment first.
It is possible that you have a 3.5.x version, but don't see these yet because it has not been configured for your project yet. I would recommend following the guide in Setting up approvals in order to set this up and working.
