Adding new fields in JIRA Service Desk Portal (i.e. form fields) when workflow status changes

I was inquiring if there is a way to display new form fields in the JIRA Service Desk portal. For example I have 5 fields for a Inquiry Request type. A portal user enters the data and submits a request. When a Service Desk agent works this request, if they move it to an approved status I want the portal user to see new fields to fill out for that inquiry. 

Is this possible?
