Basically I need to turn what used to be a paper form into a service desk request. I have about 25 custom fields I need to present when a particular service desk request is made. They could all be single or multiline input boxes. How should I go about doing this?
I don't know if there's any limit to the number of fields that you can add to a service desk request. You should just be able to add the custom fields to the create screen associated with your request type's issue type and then add those fields to the request.
