"Help us improve JIRA" form not working

I wanted to post a feature request through JIRA, but the form is not working:

It only shows me "Name" and "Email" fields and when I click "Submit" it complains that I haven't filled in "Summary" field.

PS. I don't know whether I'm using "JIRA core" or "JIRA software" - hard to tell as they're both software for sure...

Same here, I cannot see any other field in that page.

This might be a long shot but do you have some kind of Ad Blocking extension enabled in your browsers? If so, can you try disabling it and see if the issue is still there?

I tried disabling ad blocker and various other extensions, but no luck. Even when I inspect the HTML with Chrome dev tools I only see two form fields: name and e-mail. So it doesn't look like something is hidden away.
