You can create a separate project or use the same project and crate Retrospective/Review notes as stories. Actions can be crated as sub-tasks for these stories.

You just create an issue with Issue Type Story or you can add your own issue type called "Spring review".

You can also use a Confluence page to place all the insights the team ran into during the sprint. We built a macro where all your team can collaborate in real time to create the Actions Items and then assigned them to team members.
