How to create a workflow condition for issues with unreviewed code

Need to create workflow condition for issues that have code.  

  1. All code has been reviewed  - can transition
  2. Code is not reviewed  - requires a mandatory comment to transition

The 'code has been committed' option identifies the issues with code, but i have not found a way to identify issues with unreviewed code.

What exactly is the "code has been committed" and how is it being used/updated based on code reviews?
