How do I collect feedback (I want to present a form with an input) when the task is marked ready to review?

This question is in reference to Atlassian Documentation: Task management

I am using Task Management to have a simple workflow for someone to test some software.

During issue creation, I fill out the fields "Steps to complete the task" and "Expected result."  This part works well.

When the task moves through the workflow from "In progress" to "Ready to Review" then "Approved,"  I need the tester to fill out a field called "Actual result."

How do I do this?

I have never used the Task Management type of project, but in other types you can add a validator on the worfkow step you are transitioning and require a field to be filled in before allowing the issue to transition.
