Does anyone have an example Excel CSV file used for importing issues?

Here is mine. An easy way to make one:

1) pick an issue
2) export to excel
3) in excel remove the columns you don't want
4) now save it as a csv file.
5) go through the process of importing the file/issue
6) during the process it will ask you if you want to save your configuration file. Click yes and you will have your own personal configuration file. I keep my on my desktop for ease of navigation, doesn't matter where you keep it.

Hope this helps you out.
welcome
