Custom drop down in service desk

I would like to create a custom dropdown in service desk. Is it possible? Currently I see only 12 fields and I am able to select only from them.

You would need to create a custom field of the correct type (Select list). Add this to the create, edit, and view screens for the issue type that is being used by your request type. Then configure your request type, click add fields and you should see the new custom field.
