All screens in JIRA have a new 'unwanted' tab that was not originally designed, and is not removable

We recently found all screens in our JIRA Instance (Server Edition, 6.3.15) got an additional tab named "Planning" with fields that seem to be related to a Gantt Chart addon of some sort. The original Fields tab automatically gets renamed to "Standard", and a new tab named "Planning" with the additional fields gets added to every existing and new screen that is designed for a new issuetype.

We did have two expired Gantt chart addons which have since then been disabled and uninstalled. We tried creating new screens after cleaning out those old addons, but the problem persists. JIRA is STILL adding the planning tab to all newly designed screens, AND it is not editable/removable when you go into . 

We have tried to look at logs, installed plugins, no recent changes have been made to the instance, and we are trying to identify:

A) what caused this

B) how to remove the unwanted tab from the screens.

Hmm, that's interesting.  Could you look at all of your "user installed" add-ons and list what you've got in there?  (Just the installed ones, but both enabled and disabled)?

It took us a few weeks to identify the cause, an exiting employee had run a script to add additional fields and tabs to all screens. We uninstalled all obsolete add ons, and one of the deleted ones allowed us to edit the tabs and fix the additional stuff we didn't want in.
