Forms in Jira Service Management - Data Center

Hello everybody,

Is there any solution for creating forms in Jira Service Management - Data Center?

We are setting up new Service Management projects and we would like to be able to create forms. As far as i could see by now the only possible option is ProForma but frankly speaking, after acquiring Jira Service Management and Confluence, i am not very sure i'd be able to find additional budget. (the number of jira admins will be no more than 4 - the number of agents is 100)

@Mihai Marculescu there is a free version of ProForma, ProForma Lite, which allows you to build three form templates for free. Hopefully this will help you meet your immediate need and act as a proof of concept to justify the upgrade to the paid version.
