How do I setup email notifications on Fisheye?

How do I setup email notifications on Fisheye so that every time someone creates a review or mentions me in a comment, I get a email notification?

We are using git and are running Fisheye 3.8 and in my global settings and have setup my smtp config and I can send test emails successfully.

However, when folks creates reviews or mentions others in reviews, no emails get sent to developers.

Are there other settings we need to configure to make this work?
