"Unknown Attachment" after refreshing a page

I am currently confronted with more and more requests that attachments (mostly images) are no longer displayed on an existing wiki page after a change has been made on the page.

• instead of the preview of the attachment we'll find the error message "Unknown Attachment"
• however, the attachments are still attached on the page but the preview doesn't work longer
• in the change history i can exactly determine, when this happened
• it is very time-consuming to have to insert the attachments again and again

This behavior occurs only sporadically and is almost impossible to reproduce.

Does anyone of you know this problem or the root cause?
Or do you have an idea, to avoid this behavior?

There are known workarounds that may help you!
