I have created multiple tabs and added include page to each of the tabs. It displays perfectly in preview. However, when I publish it the 2nd tab displays only the word 'extension'. What is wrong here?

It appears that you have Aura Content Formatting Macros installed. You would best be suited to contact their support to understand why the behavior of macros place in the tabs is not as expected.
