Unfortunately attachment labels are handled a bit wonky in Confluence. For instance when you go to apply a label to a pge as you type it will give you suggestions that match what you are typing. Not so for attachments. Basically, all I could say is that they have not given as much thought to attachment labels as they have to page labels.
