Table of Contents macro places bullets on right side for first few headers

For the past few weeks I have noticed that in our docs (which all use the TOC macro) the first few headings in the page are displayed by the TOC macro on the right side, then the rest of the headers have the bullet on the left side. If I edit the first few headings to put in extra carriage returns or re-set the Heading level for them, this problem goes away, but then it recurs later. 

Is this a bug? Is there a workaround? We are running Confluence 5.10.3.
