We are using Confluence for employee evaluations with a 5-point rating. Is there a way to add check boxes on each task for a supervisor to check one of the 5 rating? It would be even better if it totaled it and gave the average at the end.

The easiest way is to put numbers 1-5 in your cells (for each task and for each employee). Then you'll be able to use some 3rd party apps to perform the required calculations.
We develop the Table Filter, Charts & Spreadsheets for Confluence app, and you'll be able to use its Table Filter and/or Table Transformer macro to count the required totals/average numbers.
