There a number of add-ons in the marketplace that have functionality like what you are asking about. They all have their strengths and weaknesses. Below are some examples. I Have used ConfiForms in our environment a lot and think it is an excellent add-on. The possibilities are almost limitless and the devs are great at fixing bugs and adding new features. The others ones I have heard good things about as well, I just have not used them personally.

You would probably extend the Content Properties function – https://developer.atlassian.com/confdev/confluence-rest-api/content-properties-in-the-rest-api

Sorry, I see you marked this as confluence-administrator, not confluence-developer.

I have used Adaptavist Forms the most – https://marketplace.atlassian.com/plugins/com.adaptavist.confluence.formMailNG/server/overview

But many add-ons exist.

It also seems like depending on the use-case, you might rather be looking for something like Self-Run Reports – https://marketplace.atlassian.com/plugins/org.swift.confluence.run/server/overview
