Is there a "state" variable available while editing datasets in the forms?

Hello. I'd like to know if there is such thing as a state variable available to query so when you edit existing data you could add say like a notes column asking the user on why the record is being changed. This would only show up on the edit portion of the data and not the new form input. This is for like auditing purposes so it would have to be required before allowing input of the form.

Anything like that available?
