How to use the Product Requirements blueprint?

When I create a page using the Product Requirements blueprint, the page looks nothing like the screenshot provided in the documentation (https://confluence.atlassian.com/download/attachments/329975392/RequirementsIndex_annotated.png).

For instance, the requirements list is a simple table, not connected to Jira, and there is no "Add Product Requirement" button.

You are mixing things up. Each line in tha screenshot you provided is a singe product requirements page. If you add the blueprint you automatically get the "Product Requirements" short link. This overview is the result from the screenshot...
